These forms include a Medical Certificate, a Claim form as well as confirmation that the accident was reported to the Police. 

To progress your claim quickly, these forms need to be accurate and complete. The most common issues that we see are:

• the Medical Certificate is not provided or isn't completed sufficiently
• the Claim Form hasn't been signed.
