Form	Description
Additional investment form	Use this form to make an additional investment to your WealthSmart account.
Application to update smoking status	Use this form to tell us that you have stopped smoking and change your smoking status.
Authorised representative form	Use this form to appoint another person or entity with the legal capacity as your authorised representative.
Change of adviser form	Use this form to change the advisor on your WealthSmart account.
Change of details form	Use this form to change your name, contact details, method of receiving your annual report, your pension payment bank account details, or to provide your tax file number.
Child pension details form	Use this form if you’d like to nominate either part or all of your death benefit to be paid out as a child pension to one or more of your children under age 18.
Contribution splitting application form	Use this form to split your super contributions to your spouse.
Death claim form	Use this form to notify us of the death of a WealthSmart member.
Deposit slips form	Use to form when making contributions into your account at a Suncorp branch.
Direct Debit Request form	Use this form to set up a regular contribution plan, or change your nominated bank account.
Investment change form	Use this form to change your existing or future investment strategy, or both, or to change your pension payment strategy.
Letter of compliance	This letter confirms that your Suncorp fund is a complying superannuation fund.
Lifestyle event insurance form	Use this form to apply for the Lifestyle Event Cover option.
Insurance application form	Use this form to apply for Death, Total and Permanent Disablement (TPD) or Income Protection insurance cover.
Non-lapsing death benefit nomination form	Use this form to nominate who you’d like to receive your superannuation benefits upon your death.
Proof of identity form	Use this form to complete the proof of identity requirements. You don’t need to complete this form if you’ve completed the ’Proof of identity’ section in the withdrawal form.
Reinstatement of Income Protection after leaving employer form	Use this form to reinstate your Income Protection cover attached to your WealthSmart Business Super account, after leaving your employer
s290-170 Notice of intent to deduct	Use this form if you wish to claim a tax deduction for your personal contributions.
Severe financial hardship form	Use this form to request to release superannuation monies on the grounds of severe financial hardship.
Standard choice form – WealthSmart Business Super	Use this form to have your employer direct SG contributions into your WealthSmart Business Super account.
Standard choice form – WealthSmart Personal Super	Use this form to have your employer direct SG contributions into your WealthSmart Personal Super account.
Transfer authority form	Use this form to consolidate all your other super funds into your WealthSmart account.
Withdrawal form – Super	Use this form to make a cash withdrawal from your account or transfer to another super fund.
Withdrawal form – Pension	Use this form to make a cash withdrawal from your account or transfer to another super fund.
