Form Description
Death Benefit Nomination Form Use this form to make, amend, confirm or revoke binding death benefit nominations and non-binding death benefit nominations.
Decline insurance form Use this form to cancel all insurance cover
Letter of compliance This letter confirms that your Suncorp fund is a complying superannuation fund.
Withdrawal form Use this form to make a cash withdrawal from your account or transfer to another super fund.
Change of details form Use this form to change your name, contact details, method of receiving your annual report, your pension payment bank account details, or to provide your tax file number.
Spouse application form Use this form to open an account for your spouse.
New member application form Use this form if you’ve decided you want to join the Suncorp Staff Superannuation Plan and open an account.
Investment change form Use this form to change your existing or future investment strategy, or both.
Contribution splitting application form Use this form to split your super contributions to your spouse.
Transfer authority form Use this form to consolidate all your other super funds into your Suncorp Staff Superannuation Plan account.
Conversion of defined benefit form Use this form to convert your defined benefit account to an accumulation account.
