Life Insurance Administration Forms

Form Name Description
Direct Debit Request Use this form to arrange for contributions to be periodically deducted from a financial institution account.
Application for Trauma Reinstatement Use this form to apply to add the Trauma Reinstatement Option to your existing Comprehensive Trauma Insurance policy
Non-Smokers Declaration Use this form to declare your status as a non-smoker.
Payroll Deduction Form Use this form to arrange for premiums to be periodically deducted from your salary or wages.
Tax File Number (TFN) Form Use this form to provide your Tax File Number (TFN)
Nomination of Beneficiary (Non Superannuation) Use this form to nominate a beneficiary of your policy.
Memorandum of Transfer Use this form to transfer ownership of a policy.
Application for Revival Use this form to reinstate a lapsed policy.
Death Benefit Nomination Form - Suncorp Superannuation Term Life Insurance Use to form nominate a beneficiary of your Term Life Superannuation policy.
Superannuation Cancellation Request Form Use this form to cancel Term Life Insurance that is within your Superannuation.
Cancellation Request Form Use this form to cancel your risk insurance products. If Life Insurance within superannuation use Superannuation Cancellation Request Form.
